Licensed Practical Nurse I provides direct and indirect patient care services that meet the psychological, physical and general aspects of care; utilizes the nursing process in planning care; meets the communication and education needs of the patient and family; provides care that reflects initiative and responsibility indicative of professional expectations. Communicates in a professional, confident, and assertive manner congruent with Mercy Service Standards. Communicates with physicians and other team members about changes in the patient’s condition, and can respond quickly and accurately to changes in condition or response to treatment. Additionally, can perform general nursing duties in other departments with adequate supervision. Maintains regulatory agency requirements and adheres to hospital policies, procedures, and standards.

Licensed Practical Nurse Qualifications:

• High school diploma or equivalent.
• A graduate of an accredited LPN (Licensed Practical Nurse) program.
• Current Arkansas LPN (Licensed Practical Nurse) License.
• Certification in basic life support.
• Ability to read, write and communicate effectively in English.
• Good computer knowledge required.
• At least one year of previous experience in clinical nursing preferred.
• Additional languages preferred.

Responsibilities of Licensed Practical Nurse

Provides direct patient care in a personalized and compassionate manner utilizing the Nursing Process:

• Successfully demonstrates proficiency of basic skills, equipment, and procedures, including unit-specific skills.
• Assists physicians with examinations, procedures and other processes related to patient care.
• Safely administers medication by following the established policies and procedures.
• Develops care plans initiated by RN/LPN’s (Licensed Practical Nurses) can update and resolve care plans.
• Communicates problems to the interdisciplinary team.
• Monitor alarms are set appropriately to patient status and are responded to without delay.

Documents in a manner which is complete, accurate and legible in all designated areas:

• Requests Admission Assessment and new patient education by RN when a new patient is admitted to a clinical area; requests RN observation on all assigned patients each shift and if condition changes.
• Collects data and documents condition of each assigned patient per department policies and as condition changes.
• Completes discharge instructions as per policy.
• Completes consent forms according to policy.
• Completes medication reconciliation forms per policy.
• Completes other chart forms and flow sheets appropriately.
• Charting reflects implementation of interventions from the care plan.
• Reinforces patient education and response is documented.
• Physician orders are noted, and the communication board and report sheet are updated.
• Documents medication administration as appropriate:
  • Medications are verified with EMAR and actual physician orders.
  • PCA forms are completed accurately.
  • Medications and IV’s are documented on the EMAR.
• Pain assessments are performed and documented per policy.
• Blood products are administered and documented per policy.

Communication skills as evidenced by:

• Communicates verbally and non-verbally in a concise, efficient, and appropriate manner to all hospital personnel, physicians, patients, and family/significant others:
  • Hand-off patient reports are complete, concise, and accurate utilizing SBAR.
  • Communicates with dignity and respect to all personnel, physicians, patients, and visitors.
  • Reports significant changes in patient or clinical area condition to Clinical
• Coordinator/Charge Nurse.
  • Calls physicians as needed and has all required information available.
  • Utilizes chain of command appropriately.
  • Notifies Unit Secretary or Clinical Coordinator/Charge Nurse before leaving the unit.
  • Assists in keeping the noise level to a minimum.
• Displays a positive attitude.
• Delegation: Makes safe and appropriate assignments for co-workers and role models effective communication and behavior for team members; makes appropriate requests to the team and ancillary personnel.
• Collaboration/Coordination:
  • Ensures patient needs are met, and adequate supplies are available for the oncoming shift.
  • Demonstrates cooperation and support as a team member.
  • Works with ancillary departments to schedule tests/procedures to facilitate completion.
• Education of patient and staff:
  • Acts as a change agent to facilitate new processes.
  • Evaluates knowledge and provides verbal and written instruction.
• Answers all call lights and telephones promptly and professionally. Can respond to questions or direct the caller to the appropriate personnel.

Actively supports Performance Improvement Initiatives by following hospital policies, procedures, and programs; provides ongoing peer education (i.e., Customer Satisfaction, Physician Satisfaction, Co-Worker Satisfaction, and Process Improvement):

• Reports occurrences and medication events to Risk Management using approved forms.
• Reports allergic/adverse medication reactions to Pharmacy.
• Adheres to Infection Control Policies.
• Adheres to Employee Health Policies.
• Performs record reviews and chart audits.
• Identifies, documents, and reports concerns where improvement will enhance patient care and safety.

Demonstrates behavior that is supportive of cost-effective patient care:

• Sets priorities and utilizes time efficiently to complete work on time.
• Ensures items used are adequately charged and used appropriately.
• Removes and returns discontinued equipment.
